The aim of Utilita Giving is to support individuals, families and households in England, Scotland and Wales who are experiencing fuel and/or food poverty in the form of funding, advice and grants.

Delivering a grant application form

Having researched the market for a suitable solution, Utilita Giving approached pro-Forms® to see whether our Online forms product suited its grant application process.

Starting with a dedicated, branded application form linked to the Utilita Giving web site the grant assessment process would be based on various criteria and information provided by applicants. The key requirements of the online application form included security, simplicity for applicants when submitting information and matching the required Utilita branding.

How did our Online forms work?

The process for designing the form content was extremely straightforward with Utilita providing a list of form fields, descriptions and options for dropdown and radio button fields.

Another important requirement was the ability for applicants to be able to attach/ upload documentary evidence to support their application - this could be in the form of attached PDFs and image files.

Form branding guidelines were provided along with imagery, fonts and hex colour references so they could be added to the pro-Forms® Theme option which could be saved and applied to other future forms for continuity of look & feel. The final decision Utilita needed to make was which fields to make mandatory on the form, although this sounds straightforward making all fields mandatory could cause some form respondents to abandon their application through not being able to fully comply.

On submission of a completed form, Utilita would be notified so they could retrieve the applicants' details securely from the pro-Forms® site. Custom reports could be generated containing a consolidated set of applications for specified time periods along with a PDF of each aplication recieved for vetting and further processing.

Ensuring well thought through options for the closed field type questions would also lend itself well to data analysis whether that being using the quick on-screen Data Analysis option within the pro-Forms® system or via a PowerBI visual dashboard using the dedicated pro-Forms® PowerBI connector.



The outcome

The final outcome of the form building process was to publish the form via the unique form link from an easy to access button on the Utilita Giving website.

Other options available for publishing the form include a dedicated QR code, embedding the form with in a iframe of the website or publishing it to a Twiiter social media account.
